Dear Student,
At Crossroads, we are committed to protecting the privacy and confidentiality of your health information. This Notice outlines how we may use and disclose your Protected Health Information (PHI) and details your rights concerning this information.

We understand the importance of your health information and are dedicated to safeguarding it. This Notice applies to all records of your care generated by our organization.
- Uses and Disclosures of Health Information
We may use and disclose your PHI for the following purposes:
- Treatment
To provide, coordinate, or manage your healthcare services. For example, our therapists and staff may share information to develop the most appropriate care plan for you. - Payment
To bill and collect payment for the services you receive. This may involve sharing information with your insurance company to obtain authorization or payment for treatment. - Healthcare Operations
For activities necessary to run our programs and ensure quality care. This includes staff training, quality assessments, and evaluating the performance of our services.

As a client, you have the following rights regarding your PHI:
- Right to Access
You may request to inspect or obtain copies of your health records. To do so, please submit a written request to our Office Manager or call (801) 369-0238 . - Right to Amend
If you believe your health information is incorrect or incomplete, you can request an amendment. Submit a written request stating the reason for the amendment to our Office Manager or call (801) 369-0238. - Right to Request Restrictions
You may request restrictions on certain uses or disclosures of your PHI. While we are not required to agree to all requested restrictions, we will consider your request carefully. - Right to Confidential Communications
You can request that we communicate with you through alternative means or at alternative locations. We will accommodate reasonable requests. - Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures of your PHI made by us. This excludes disclosures made for treatment, payment, healthcare operations, and other exceptions. - Right to a Copy of this Notice
You may request a paper copy of this Notice at any time.

We reserve the right to change this Notice and make the new provisions effective for all PHI we maintain. The revised Notice will be available upon request and on this page.

Student Confidentiality & Rights
It is the intent of the Crossroads Academy Inc (CRA) organization to maintain strict confidentiality concerning its students in compliance with Code Section 42 of the federal regulations and state statute. Failure to maintain confidentiality of student information, such as, disclosure of student’s identity, physical whereabouts, or student’s status is prohibited and unlawful and may be punishable by fine or other court action. We request (per statute and federal regulations) that CRA do not discuss other students/families or issues with anyone outside of the staff at CRA.
Confidentiality Exceptions
By law, there are certain incidences where confidentiality will be overridden in order to report to the proper authorities, these include:
- If a student reports abuse, a call to Student Protective Services must be made within 24 hours of the report. It is not the responsibility or right of CRA to investigate or to determine guilt or innocence. It is only CRA's responsibility to report the alleged abuse.
- If a student discloses plans to hurt, kill, or abuse someone or themselves, CRA does have the “Duty to Warn” the targeted victim. This may be done verbally or in writing.
HIPAA Regulations: CRA acknowledges that CRA complies with the Health Insurance Portability and Accountability Act of 1996.
Student Rights
Students (clients) have the right to:
- be treated with dignity.
- be free from potential harm or acts of violence.
- be free from discrimination.
- be free from abuse, neglect, mistreatment, exploitation, and fraud.
- privacy of current and closed records.
- communicate and visit with family, attorney, clergy, physician, counselor, or case manager, unless therapeutically contraindicated or court restricted.
- be informed of agency policies and procedures that affect client or guardian’s ability to make informed decisions regarding client care, to include:
- program expectations, requirements, mandatory or voluntary aspects of the program.
- consequences for non-compliance.
- reasons for involuntary termination from the program and criteria for re-admission.
- program service fees and billing.
- safety and characteristics of the physical environment where services will be provided.
Providers:
- Each provider shall maintain, in the client file record, a copy of the client’s rights, signed by each client or client’s guardian.
- Each provider shall prominently display a poster that notifies clients of their rights.
- Each provider staff shall be trained annually on this rule. Each provider shall ensure that each staff personnel file contains documentation of training completion which shall be individually signed and dated by the trainer and staff.
